It can be preferred to collect informed consent forms (ICFs) digitally, so we can easily store them somewhere safe (MLN Lab stores them in a separate Project Storage Drive).
To achieve this, a straightforward way is to use Adobe Sign.
- Go to Adobe.com
- Log in with TU Delft email. Click business / school account. Then just fill out TUDelft credentials
- Find the apps in the top-right corner, click Adobe Sign
- It might ask you to sign in again. Sign in again using TU Delft account.
You should just need to click the link in the email and follow the instructions.
If you need to send it only once, just go straight to fill & sign. If you need to re-use it (as with an ICF), it’s best to create a template.
Create Template
- Click create a re-usable template.
- If you used the TU Delft Informed consent form templates (but also with most other documents), it will tell you it found fields, just allow the program to fill them in. Mostly this works well enough.
- Check if all the fields are correct. If they are not, manually correct them (Double click, or RMB–>Edit, repeat for all incorrect fields).
- Make sure to set the fields that are to be filled by the researcher (you), as a different participant (e.g. participant 2).
- Click save.
Send Template
- In adobe sign, click Send
- Add the recipient(s) that need to sign it, then add yourself. Also, disable the order requirement, just delays things.
- Select the template (if you made one).
- As a message, add something like:
Please read this informed consent form carefully and thoroughly. If you wish to consent, please sign this form before the start of the experiment.
- Check the fields and correct if needed, and send.
